As you can see in the screenshot below, we were making changes to a clients anti spam policies in exchange online when we were prompted with:

The problem is that this Microsoft 365 Tenant has been in use for years so we had no idea what was REQUIRED to be configured and when we clicked YES, nothing would happen for several minutes and then this popup would appear:
Sorry, we couldn’t update your organizational settings. Please try again.
The fix was to setup Multi-Factor Authentication and then make the Exchange Online Anti-Spam Policy change.
- surf to https://myaccount.microsoft.com
- click SECURITY INFO from the menu on the left
- click ADD SIGN IN METHOD
- select AUTHENTICATOR APP from the drop down
- complete the MFA setup
- try your Azure or M365 change again
- have a nice day
